Dual Credit Enrollment
Detailed Enrollment Instructions
Contact your Guidance Counselor to request permission to dual enroll and the following forms:
a UF Form 542 (also called the Textbook Form) (SBAC students only)
a Student Support Services Dual Enrollment Application for UF
(SBAC students only)
Contact the Center for Precollegiate Education and Training (CPET) at (352) 392-2310 to sign up for a UF Dual Enrollment workshop and to request:
Print, complete and submit dual enrollment forms prior to deadline.
UF Non-Degree Registration Form
UF Immunization and Health History Form (only required once)
UF Form 542 (also called the Textbook Form) (Private and PK Yonge students)
Dual Enrollment Application
Complete all necessary forms in their entirety and mail to the dual enrollment office at CPET.
Attend the UF Dual Enrollment Workshop you signed up for to begin the pre-registration process. Parents are welcome to come with you, but may not attend in lieu of the student.
Attend academic advisement appointment with the UF Honors Office (129 Tigert Hall). This appointment is scheduled by the CPET office when you call to sign up for one of the dual enrollment workshops.
Each student attends his/her own individual UF advising appointment.
Missed appointments cannot be rescheduled.
**Please bring a copy of your current high school transcript and a list of courses from the
course schedule with you to your advising appointment.
Confirm receipt of immunization forms - You MUST meet UF’s immunization requirement prior to registration. If you have questions regarding the immunization form or requirements, you must contact the Student Health Care Center at (352) 392-1161.
After you have registered for the semester, you may obtain your textbooks (using the Form 542) from the University of Florida Bookstore located in the J. Wayne Reitz Union on campus. Please fully complete the Form 542 for each textbook, including title of book, new/used status, publisher, and cost. Though tuition fees are waived, private and homeschooled students are responsible for the costs of textbooks and instructional materials for dual enrollment courses.
Alachua County students: All books purchased with the Form 542 must be returned to the University of Florida Bookstore by the end opf the semester.
